Instructions to complete the Ride Along Waiver:
1. Download the Adobe Acrobat Reader software to ensure you are fully able to fill, electronically sign, save and submit the Waiver. (There are many other PDF readers and most web browers like Google Chrome, Firefox, and Microsoft Edge will allow you to open and view a PDF but these web browser PDF viewers will not allow you to sign the document and save the completed document to your local computer.) We recommend you using the Adobe Acrobat Reader software for completing these applications. Click the link to the right to download a free version of the software.
2. Click the link above for the Ride Along Waiver, the Waiver will automatically download, and save to your local computer. (You must save to your local computer to sign and save a completed copy that can be uploaded in Step 2.)
3. Fill and complete the Waiver, the last step will be to electronically sign. (Once you electronically sign the document you will not be able to make any additional changes to the application. Please ensure that all fields are completed before you elctronically sign.)
4. Save the final electronically signed document copy of the Waiver on your local computer.
As part of the online request form you will be required to upload the signed and completed Waiver from Step 1.
